Formulary Committee (FC) accepts application forms from prescribers and pharmacists, to consider medicines for approval of prescribing in NHS Lothian.
FORMULARY APPLICATION FORM 1 (FAF1)

This form is for medicines that have been considered and approved by the Scottish Medicines Consortium (SMC.)
This form is completed to allow the medicine to be approved for local use. It can also be used to indicate that the medicine should not be routinely used in Lothian, as there are suitable medicines available that are preferred for use by local clinicians.
FORMULARY APPLICATION FORM 2 (FAF2)
GUIDANCE FOR COMPLETION OF FAF2 or FAF3
This form is for medicines that are being used in an 'off-label' indication or for unlicensed medicines. A FAF3 forms should be submitted to the appropriate Drugs and Therapeutics Committee, for support prior to being submitted to FC.
The ADTC 'NHS Lothian Policy and Procedures for the Use of Unlicensed Medicines' should be referred to for completion of FAF3s.
To request an amendment to the Lothian Joint Formulary a Formulary Amendment Request Form should be completed (see appendix 1 of the Adult Formulary for more information).
